Independent Contractors,
Here you will find all of the materials that you need in order to remain qualified with the metro-area Support Services Brokerages. You will need to compile all of the materials on the Requalification Checklist, and bring the completed packet to one of the Requalification Sessions that are scheduled throughout the months of May and June.
There are documents in the packet that require you to read and sign, as well as a weigh test that requires you to provide answers to a set of questions as they pertain to your business and business practices. Please review the Independent Contractor Weigh Test (Word Version here; PDF version here) to assist you in answering the questions from the form. There are also items on the checklist that require you to provide outside information to the packet, including liability insurance, copies of identification documents, reference letters, etc.
All of these elements must be present in the completed packet in order to maintain qualification. Please spend some time reading the updated IC Guidelines, as some rules and practices may have changed since you went to Orientation.

The Qualification Sessions will be structured in a way that you do not need to schedule an appointment, and you can plan to simply drop off your packet at these sessions. Provider Coordinators will be available during these times to answer any questions that you may have about the process, or about providing services in general should you wish to sit down with someone.
